Google Forms
Google Forms is an online tool that allows for the quick and easy creation of online surveys, questionnaires or polls. Creating a form does require a Google account, but once this done creating forms is an easy process. Creating surveys involves writing questions and determining a response method (eg. Short answer, scale). Once the survey has been completed it can be sent out through email or by accessing the link to the survey. Responses can be accessed immediately, and can be shown either individually or a summary which shows results in simple and easy to read bar graphs. Below is an example of a Google Form I created for my SGA in response to our lesson and ICT tool.

In my opinion I can't find many negatives with Google Forms, possibly the only negative I have found is that if you are going through responses as a class it may be more beneficial to answer one question, go through the answers and move on to the next instead of doing them all as a group. This is however an extremely minor negative, in comparison to the benefits it has.
Google Forms can be used at many different times in the classroom. A few examples include gathering information on student prior knowledge, doing a reflection on an activity that has just been completed or doing a poll on where to do the next class excursion.
Google Forms would be in the augmentation category of the SAMR model as it acts as an substitute to traditional paper survey however it improves it by being able to receive the responses immediately. For the Bloom's taxonomy it would be in the analyse category.
